CPD Workshop forms
Venue for all workshops unless otherwise stated is: GP Access Head Office Training Room 1, 1st floor 123 King Street, Newcastle.
Participants are required to register for workshops via the official registration form. These forms are attached below. They maybe emailed to workshops@gpaccess.com.au or faxed to 49252268.
The attendee is required to pay by credit card or cheque/money order in advance unless their Practice is approved for direct billing. The practice will then be invoiced after the workshop. If practice is paying by EFT transfer, please contact Amy Harrison with particulars. aharrison@gpaccess.com.au or 49260534.
